Your New Patient Checklist
We’re thrilled you’ve taken the first step on your fertility journey with Fertility Partnership. To help make your first consultation as productive and informative as possible, we ask that you complete a few key steps before your appointment. Completing these in advance ensures that your consultation with Dr. Elan Simckes is personalized, accurate, and tailored to your goals.
1. Pre-Appointment Testing
Before your consultation, we ask that patients provide the following:
Blood Work
- Purpose: Provides critical information for creating an accurate treatment plan.
- Timing: Ideally collected 2 weeks before your appointment.
- When to Schedule: Samples should be taken on day 3 of your menstrual cycle (the third day of bleeding). This timing is important because certain hormone levels, including FSH and estradiol, fluctuate throughout the cycle. Day 2 or 4 samples may also be acceptable. If you are on any hormone therapy, this test is not recommended until therapy has been paused.

Where to Get Tested:
- Fertility Partnership “Walk-In” Lab: Monday–Friday, 8:00–11:00 AM at 5401 Veterans Memorial Pkwy Ste 201, St Peters, MO 63376. Get directions here.
- Any Labcorp Location Near You: Women’s Fertility Ovarian Reserve Test. Purchase your test online here and find a nearby Labcorp location here.
- Cost: Covered by insurance or self-pay (~$175–$219). Self-pay orders can go directly to Labcorp.
Note: If your blood work has been completed in the last 3 months, you do not need to repeat it. Please bring a copy of your previous blood work if you have it.
Semen Analysis
- Purpose: Provides essential information about male fertility factors to guide your treatment plan.
- Method: Completed via the Fellows’ At-Home Testing Kit. Purchase online here.
- Timing: Samples should be provided 2 weeks before your scheduled appointment.
- Cost: $199
Previous Results: Acceptable if done within the last 6 months. Please bring a copy of your previous results if you have it.
2. Preparing for Your Appointment
- Make sure all required blood work and semen analysis are completed at least 2 weeks prior to your appointment.
- Bring any prior fertility testing results, including blood work, semen analysis, or imaging.

3. Questions & Special Considerations
- Patients with PCOS: If you have polycystic ovary syndrome (PCOS), you may refrain from completing blood work prior to your appointment. Our team will provide personalized instructions as needed. Please reach out if you have any questions.
- Insurance & Billing: Blood work can be billed to insurance or paid out-of-pocket. Semen analysis is self-pay only.
